Abstract
:
This commissioned study includes two Phases. The main objectives of Phase 1 are to recommend service models to (i) integrate health care services for elderly people, (ii) enable elderly people with stable long-term conditions to receive healthcare in their own environment and to stay in their own environment, and (iii) enable elderly people with more complex health needs to live and/or die in place. Phase 2 is to pilot the service model(s) developed to test applicability and impact. A study framework which takes into consideration 3 dimensions, i.e. (i) the clinical care and social needs of patients, which (ii) generate the demand for primary, secondary and community care services, and (iii) the organization of health and social care will be adopted in the study. An Expert Panel will be formed to advise on the conduct of the study. Phase 1 includes a review of healthcare services supporting the elderly, and a mix of qualitative and quantitative methodology including (i) secondary analysis of hospital data, (ii) key informant interviews with policymakers and service providers, (iii) focus group discussions among health and social health professionals, (iv) case study, and (v) questionnaire surveys among the elderly and general public to identify factors and barriers in providing care to the elderly, and to recommend changes including legislation if required and measures to foster a community culture to facilitate the implementation of the above recommended service model(s). A systematic evaluation of implementation fidelity and an outcome/impact evaluation will be conducted in Phase 2 to test the applicability and impact of the service model(s) being piloted. We will also construct a System Dynamics Model to provide additional insights in the development of service model(s) for the elderly in Phase 1 as well as to study the impact of the service model(s) in Phase 2 by processing the qualitative and quantitative data in a simulation modelling.
